| Once Reactivation Fee is received, a new active CPD cycle will be initiated. Within 30 days, a new CPD cycle invoice will be provided for submission of payment and point requirements for this cycle. |

| If you need an invoice, select the "check" payment option during checkout. After completing the checkout process you will immediately receive an "order received" email which may be used as an invoice for payment. The email will also provide instructions on where to send the payment. |
